A transition program for families coming home after a NICU or PICU stay. We continue the specialized neonatal feeding management the hospital team started, and we follow closely during the critical first 6 months. We coordinate with your pediatrician and specialists to ensure continuity.

How it works

THE PROCESS.

  1. 1Comprehensive review of hospital discharge plan
  2. 2Initial visit within first 2 weeks of returning home
  3. 3Weekly or biweekly sessions as needed
  4. 4Continuous coordination with medical team

  • Your baby was discharged with a specialized plan
  • There were feeding difficulties during hospitalization
  • Close growth monitoring is needed
  • They have support equipment (tube, ventilation) at home
